    Summary

Internal bleaching of teeth 11 and 12 led to external root resorption and subsequent implant failure. Ridge reconstruction was challenging but ultimately successful, highlighting difficulties in restoring pre-extraction contours after adjacent tooth loss.

    Area of Science:

    • Dentistry
    • Oral Surgery
    • Periodontics

    Background:

    • A 45-year-old woman sought internal bleaching for anterior teeth (sites 11 and 12).
    • Failure to use an internal barrier during internal bleaching is a critical procedural omission.

    Observation:

    • Tooth 11 developed external root resorption, necessitating extraction and immediate implant placement.
    • Tooth 12, though initially salvageable, was also extracted; its implant subsequently failed, causing a severe ridge defect.
    • Multiple reconstructive surgeries were unsuccessful, worsening the defect to a Class III classification.

    Findings:

    • Successful ridge reconstruction was achieved by a periodontist, enabling prosthetic restoration.
    • A final restoration utilized the implant at site 11 with a cantilevered pontic for site 12.

    Implications:

    • This case underscores the complexity of ridge reconstruction and restoring pre-extraction morphology after adjacent tooth loss.
    • Highlights the importance of procedural protocols, such as using internal barriers, in internal bleaching to prevent complications.
