Abstract

Insights

Micronutrient deficiencies (MNDs) are common in Spondyloarthritis (SpA) patients, particularly affecting females. These deficiencies are linked to disease activity and metabolic issues, underscoring their role in inflammatory dysmetabolism.

Area of Science:

  • Rheumatology
  • Clinical Nutrition
  • Biochemistry

Background:

  • Micronutrient deficiencies (MNDs) are prevalent in chronic inflammatory diseases, often correlating with a pro-inflammatory state and comorbidities.
  • Spondyloarthritis (SpA) is a group of inflammatory diseases where MNDs have not been specifically investigated.
  • Understanding MNDs in SpA is crucial for managing associated health complications.

Purpose of the Study:

  • To determine the prevalence of anemia, iron (Fe), vitamin D [25(OH)D], vitamin B12 (B12), and folic acid (FA) deficiencies in SpA patients.
  • To explore the relationship between MNDs and factors such as age, gender, and metabolic abnormalities in SpA.
  • To investigate the association of MNDs with disease activity and functional status in SpA.

Main Methods:

  • A cohort of 220 SpA outpatients (137 females, 83 males) with psoriatic arthritis (PsA) or non-psoriatic SpA were evaluated.
  • Blood levels of hemoglobin (Hb), ferritin (Fe), 25(OH)D, B12, and FA were measured.
  • Disease activity (DAPSA/ASDAS-CRP), functional status (HAQ-S), and metabolic parameters including Body Mass Index (BMI) were assessed.

Main Results:

  • Anemia was observed in 13.6% of patients, predominantly females. Females also exhibited higher Fe deficiency and lower Fe levels.
  • Vitamin D deficiency (≤20 ng/ml) was found in 23.2% of patients, more common in males and inversely correlated with disease duration in males.
  • Moderate/high disease activity was linked to higher BMI, HAQ-S, and lower Hb and Fe levels. Lower B12 levels were noted in patients with extra-articular manifestations.
  • Folic acid deficiency was frequent (22%) and often co-occurred with B12 deficiency.

Conclusions:

  • SpA patients exhibit a significant prevalence of MNDs, with notable associations with gender and metabolic abnormalities.
  • MNDs play a role in the inflammatory-dependent dysmetabolism observed in SpA.
  • These findings highlight the importance of screening for and managing MNDs in SpA patients to improve outcomes.

Inflammatory Bowel Disease III: Diagnostic Studies and Management I-Nutritional Therapy

Various diagnostic tests are employed in the diagnostic process for Inflammatory Bowel Disease (IBD), particularly to differentiate between Crohn's disease and ulcerative colitis.

A colonoscopy is the definitive screening test, distinguishing ulcerative colitis from other colon diseases with similar symptoms. During a colonoscopy test, inflamed mucosa with exudate ulcerations can be observed, and biopsies are taken to determine the histologic characteristics of the...

Autoimmune Disorders

Autoimmune diseases are a group of disorders in which the body's immune system mistakenly attacks its own cells, tissues, and organs. This results from an overactive immune response against substances and tissues normally present in the body. Let's delve into the concept and mechanism of autoimmune diseases from an immune system point of view, explore different causes and examples of such diseases, and discuss potential solutions.

Fungal Phylum Microsporidia

Microsporidia are a group of obligate intracellular fungi that were initially classified as protists but were later reclassified based on phylogenetic, molecular, and structural evidence linking them to the Chytridiomycota. These unicellular, non-motile organisms are highly specialized parasites that infect a wide range of animal hosts, including humans. They have evolved extensive genomic and metabolic reductions, making them highly dependent on their hosts for survival.Morphology and Genomic...

The JAK-STAT Signaling Pathway01:20

The JAK-STAT Signaling Pathway

Several cytokine receptors have tightly bound Janus kinase or JAK proteins attached at their cytosolic tail. Small signaling molecules such as cytokines, growth hormones, or prolactins bind to the cytokine receptors and initiate their dimerization. The dimerization brings the cytosolic JAKs together that trans-phosphorylate and activates each other. The activated JAKs now phosphorylate cytosolic tails of the cytokine receptors, which serve as binding sites for adaptor proteins such as  SH2...

Chronic Kidney Disease II: Clinical Manifestations

Chronic Kidney Disease (CKD) progressively impairs multiple body systems due to the accumulation of uremic toxins, which disrupt cellular functions across various organs.Neurologic symptomsNeurologic symptoms often arise early in CKD, as uremic toxin buildup drives changes in cognitive and motor functions. Patients frequently experience fatigue, headache, confusion, difficulty concentrating, and, in severe cases, seizures. Peripheral neuropathy commonly manifests as burning sensations in the...

Immunodeficiency Diseases01:25

Immunodeficiency Diseases

Immunodeficiency disorders are conditions in which the immune system's ability to fight infectious disease and cancer is compromised or entirely absent. The immune system comprises a complex network of cells, tissues, and organs that work together to protect the body from potentially harmful invaders. When this system is deficient or not functioning properly, it leaves the body susceptible to infections, diseases, or other complications.
There are three main causes of immunodeficiency...
