Summary

Pediatric optic neuritis (ON) outcomes show significant visual acuity improvement within 6 months, maintained over two years. However, associated autoimmune diagnoses are common, with 18% experiencing additional ON episodes.

Background:

  • Pediatric optic neuritis (ON) is a rare condition lacking comprehensive characterization.
  • The Pediatric ON Prospective Outcomes Study (PON1) was initiated to address this knowledge gap.

Purpose of the Study:

  • To evaluate visual acuity (VA) outcomes in children with ON.
  • To assess recurrence risk and final diagnoses 2 years post-enrollment.

Main Methods:

  • A nonrandomized observational study conducted across 23 pediatric ophthalmology/neuro-ophthalmology clinics.
  • 28 children (age 3-15) completed the 2-year follow-up.
  • Treatment was at the investigator's discretion.

Main Results:

  • Most children achieved age-normal high-contrast VA (79%) and 20/20 or better vision (66%) by 2 years.
  • Final diagnoses included isolated ON (39%), MOG-AD (29%), MS (14%), NMOSD (11%), and ADEM (7%).
  • Recurrent ON occurred in 7% (all MS), and 7% had new episodes in the unaffected eye.

Conclusions:

  • Children with ON show substantial VA improvement by 6 months, sustained over 2 years.
  • Neurologic autoimmune diagnoses are frequently associated with pediatric ON.
  • 18% of participants experienced additional ON episodes (relapses or new eye involvement).
