Abstract

Insights

Sildenafil did not prevent cancer therapeutics-related cardiac dysfunction (CTRCD) in patients receiving anthracyclines. This randomized trial found no significant benefit of sildenafil in preserving left ventricular ejection fraction (LVEF) or reducing CTRCD incidence.

Area of Science:

  • Cardiology
  • Oncology
  • Pharmacology

Background:

  • Animal studies suggested 5-phosphodiesterase inhibitors protect against anthracycline-induced cardiac dysfunction.
  • Cancer therapeutics-related cardiac dysfunction (CTRCD) is a significant concern with anthracycline chemotherapy.

Purpose of the Study:

  • To evaluate the clinical efficacy of sildenafil in the primary prevention of CTRCD in humans.
  • To assess sildenafil's effect on left ventricular ejection fraction (LVEF) in patients undergoing anthracycline therapy.

Main Methods:

  • A randomized, double-blind, placebo-controlled trial involving 52 patients.
  • Intervention group received sildenafil 25 mg twice daily; control group received placebo.
  • Echocardiography, cardiac MRI, hs-troponin I, and NT-Pro-BNP measurements were used to assess cardiac function over a 6-month follow-up.

Main Results:

  • Both groups experienced a significant reduction in LVEF (p<0.003 for intervention, p<0.001 for control).
  • No significant difference in LVEF reduction was observed between the sildenafil and placebo groups (p=0.26).
  • The incidence of CTRCD was similar in both groups (41.6% intervention vs. 42.8% control, p=0.51).

Conclusions:

  • Sildenafil does not appear to be beneficial for the primary prevention of anthracycline-induced cardiac toxicity.
  • Further research may be needed to explore alternative strategies for preventing CTRCD.

Treatment for Pulmonary Arterial Hypertension: Phosphodiesterase Inhibitors

Phosphodiesterase 5 (PDE5) inhibitors are potent enzymes that function to hydrolyze cyclic nucleotides to their corresponding 5' monophosphates. Their unique biochemical properties have been applied in treating Pulmonary Arterial Hypertension (PAH).
Among the PDE5 inhibitors, sildenafil (Revatio) stands out as a competitive and selective inhibitor. It operates by elevating cellular levels of cGMP and augmenting signaling through the cGMP-PKG pathway, promoting vasodilation. Upon oral...

Clinical Trials: Overview

Clinical development focuses on how the drug will interact with the human body and encompasses four key phases of clinical trials, each serving a specific purpose in assessing the safety and effectiveness of new drugs. These phases overlap and build upon one another. Phase I involves a small group of healthy volunteers (typically 20-80 individuals) or, in cases where significant toxicity is expected, patients with the targeted disease, such as cancer or AIDS. The volunteers are tested for...

Clinical Trials01:16

Clinical Trials

Clinical trials are prospective experimental studies conducted on humans to determine the safety and efficacy of treatments, drugs, diet methods, and medical devices. Using statistics in clinical trials enables researchers to derive reasonable and accurate conclusions from the collected data, allowing them to make wise decisions in uncertain situations. In medical research, statistical methods are crucial for preventing errors and bias.
There are four phases in a clinical trial. A phase one...

Coronary Artery Disease IV: Preventive Measures

Effective preventive measures for coronary artery disease (CAD) focus on controlling modifiable risk factors, including cholesterol abnormalities and lifestyle changes.Cholesterol ManagementFirst, the Mediterranean diet and the American Heart Association advocate for maintaining low-density lipoprotein (LDL) cholesterol levels below 100 mg/dL, with a more stringent recommendation of below 70 mg/dL for individuals at high risk. LDL cholesterol, often termed "bad cholesterol," can lead to the...

Treatment for Pulmonary Arterial Hypertension: Receptor Tyrosine Kinase Inhibitors and Calcium Channel Blockers

Receptor tyrosine kinase inhibitors (TKIs) and calcium channel blockers (CCBs) are two critical categories of drugs employed in the treatment of pulmonary artery hypertension (PAH). PAH is a disease that causes high blood pressure in the pulmonary arteries, resulting in chest pain, fatigue, and shortness of breath.
TKIs, such as imatinib (Gleevec), are particularly effective in tackling the growth and mitogenic factors that become upregulated in PAH patients. These factors contribute to the...

Cardiac Catheterization I: Pre-Procedure Overview

Cardiac catheterization is an invasive diagnostic technique used to identify and evaluate structural and functional diseases of the heart and major blood vessels. This technique diagnoses congenital heart disease, coronary artery disease, valvular heart disease, and coronary spasms and assesses ventricular function. It helps guide treatment decisions, including the need for revascularization procedures like percutaneous coronary intervention (PCI) or coronary artery bypass grafting (CABG) and...
