Background:

  • Thyroid cancer (TC) necessitates meticulous surgical intervention, including total or near-total thyroidectomy and central lymph node dissection (CLND).
  • Optimizing lymph node yield and preserving parathyroid gland (PG) function are critical challenges in TC surgery.

Approach:

  • A systematic literature review and meta-analysis were conducted, examining randomized controlled trials and non-randomized controlled trials.
  • Data from 1870 patients were analyzed to compare outcomes with and without the use of carbon nanoparticles (CNs) during CLND.

Key Points:

  • CN application significantly increased the number of central lymph nodes and metastatic lymph nodes dissected.
  • The use of CNs was associated with a lower rate of accidental parathyroidectomy and reduced incidence of transient hypoparathyroidism and hypocalcemia.
  • No significant differences were observed in rates of permanent hypoparathyroidism, permanent hypocalcemia, or transient recurrent laryngeal nerve (RLN) paralysis.

Conclusions:

  • Carbon nanoparticles enhance the effectiveness of lymph node dissection in thyroid cancer surgery.
  • CNs demonstrate a protective effect on parathyroid glands and their function during thyroidectomy with CLND.
  • Further research may explore long-term functional outcomes and refine CN application techniques.
