    Summary

    Orthotopic liver transplantation can be safely performed without venous bypass, demonstrating good patient survival rates. This approach avoids the need for routine bypass in liver transplant surgery.

    Main Results:

    • Hemodynamic alterations observed during the anhepatic phase (e.g., decreased blood pressure, cardiac output) resolved promptly after revascularization.
    • Significant intraoperative transfusion requirements were noted, particularly in patients with prior surgery or retransplants.
    • No postoperative renal function deterioration or need for hemodialysis was observed.
    • Achieved 30-day survival of 87.8%, 90-day of 80.5%, and one-year actuarial survival of 68.8%.

    Conclusions:

    • Venous bypass is not a routine necessity for orthotopic liver transplantation.
    • Hemodynamic stability can be maintained without bypass, with prompt recovery post-reperfusion.
    • The study supports the feasibility and safety of OLT without routine venous bypass.
