- The fracture was treated via an open deltopectoral approach with screw fixation.
- Post-operative immobilization was followed by a structured physiotherapy regimen.
- The patient achieved full range of motion and strength within three months.

- Early and accurate diagnosis of lesser tuberosity avulsion fractures is essential to prevent malunion and functional loss.
