Summary

Malignant peritoneal mesothelioma (MPM) can mimic gynecological cancers, presenting with abnormal uterine bleeding. Immunohistochemical stains are crucial for accurate diagnosis, differentiating MPM from ovarian cancer and preventing misdiagnosis.

Background:

Malignant peritoneal mesothelioma (MPM) is a rare cancer.
MPM can present with symptoms mimicking primary gynecological malignancies, such as ovarian cancer.
Accurate differential diagnosis of MPM is challenging for clinicians and pathologists.

Observation:

A rare case of MPM presented with abnormal menstrual bleeding due to uterine infiltration.
The clinical presentation was initially indistinguishable from a primary gynecological malignancy.
Immunohistochemical staining was essential for definitive diagnosis.

Findings:

Mesothelial markers (calretinin, CK5/6, WT-1, D240) and epithelial markers (Claudin-4, BerEP4, B72.3, PAX-8) were utilized.
Histopathological analysis confirmed MPM with diffuse uterine infiltration.
The diagnostic challenge highlights the need for specific markers to differentiate MPM from other malignancies.

Implications:

This case underscores the importance of considering rare diagnoses like MPM in gynecological presentations.
Accurate immunohistochemical analysis is critical to avoid misdiagnosis and ensure appropriate patient management.
Improved diagnostic strategies are needed to enhance the early detection of MPM.
