Background:
- Long-standing type 2 diabetes mellitus can present with neurological complications.
- Spinal tumors can cause significant neurological deficits.
Observation:
- A 48-year-old female with type 2 diabetes presented with acute bilateral lower limb weakness.
- Examination revealed paraplegia, sensory loss to T10, and reduced reflexes.
- Imaging identified a thoracic spinal mass compressing the spinal cord.
Findings:
- Histopathology confirmed a giant cell tumor of the spine.
- Ectopic parathyroid adenoma was identified, leading to Brown tumor.
- Surgical decompression and parathyroidectomy were performed.
- Biochemical normalization of calcium and parathyroid hormone occurred post-surgery.
Implications:
- Giant cell tumors of the spine require prompt diagnosis and management.
- Ectopic parathyroid adenomas can mimic other conditions and cause significant metabolic disturbances.
- Despite treatment, severe neurological deficits may persist, highlighting the importance of early intervention.
