Background:

Pulmonary hamartomas (PHs) are typically diagnosed with difficulty, especially large cystic-solid variants.
Few large cystic-solid PH cases have been documented in medical literature.

Observation:

A 53-year-old asymptomatic woman presented with a large, flat left thoracic cavity tumor on CT.
The lesion featured multiple air-containing cysts, abundant fat, lamellar soft tissue, and a vascular pedicle to the lung.
Histopathology confirmed PH, with cysts attributed to entrapped respiratory epithelium.

Findings:

This case highlights the imaging characteristics of a large cystic-solid pulmonary hamartoma.
CT revealed a complex lesion with both cystic and solid components, including fat and vascularity.
Histological analysis confirmed the diagnosis and elucidated the cyst formation mechanism.

Implications:

Radiologists should consider large cystic-solid PH in differential diagnoses for complex thoracic lesions.
Understanding the imaging features, formation mechanism, and histopathology aids in accurate diagnosis.
Increased awareness can improve diagnostic accuracy and patient management for rare pulmonary hamartomas.
