A case study shows that SARS-CoV-2 infection can trigger inflammatory arthritis. A 38-year-old female developed rheumatoid arthritis (RA) symptoms post-infection, confirmed by positive antibodies and imaging.
Area of Science:
- Rheumatology
- Infectious Diseases
- Immunology
Background:
- Emerging evidence suggests a link between SARS-CoV-2 infection and inflammatory arthritis.
- Understanding post-viral autoimmune manifestations is crucial for patient management.
Observation:
- A 38-year-old female presented with polyarthralgia one month after SARS-CoV-2 infection.
- Clinical examination revealed synovitis in the hands and wrists.
- Laboratory tests showed positive antinuclear antibody (ANA), rheumatoid factor (RF), and anti-cyclic citrullinated peptide (CCP) antibodies.
Findings:
- Hand MRI confirmed synovitis in metacarpophalangeal and proximal interphalangeal joints, wrist joints, and tendinitis with tenosynovitis.
- The patient was diagnosed with seropositive nonerosive rheumatoid arthritis (RA).
Implications:
- This case highlights SARS-CoV-2 as a potential trigger for rheumatoid arthritis.
- Early diagnosis and treatment with NSAIDs and methotrexate led to symptom improvement.
- Further research is warranted to elucidate the mechanisms of post-COVID-19 arthritis.
