    Background:

    • Carcinosarcoma of the urinary bladder is a rare malignant neoplasm.
    • It presents a diagnostic challenge due to its biphasic nature, combining epithelial and mesenchymal components.

    Purpose of the Study:

    • To review the clinical and pathological features of urinary bladder carcinosarcomas.
    • To analyze treatment outcomes and prognostic factors for this rare tumor type.

    Main Methods:

    • Retrospective review of clinical and pathological data.
    • Inclusion of three new cases and 35 previously reported cases of urinary bladder carcinosarcoma.

    Main Results:

    • Tumors occurred predominantly in men (3:1 ratio) aged 33-82 years (average 62).
    • Common presentation included short-duration hematuria; tumors were typically large and polypoid.
    • Histologically, tumors showed admixture of carcinomas and sarcomas, with 25-50% exhibiting skeletal muscle, osseous, or cartilaginous differentiation.
    • Outcomes included 16 deaths, 4 with metastatic disease, and 12 disease-free survivors (5 months to 7 years post-op).

    Conclusions:

    • Urinary bladder carcinosarcoma is an aggressive tumor with a poor prognosis for many patients.
    • Radical cystectomy, sometimes combined with radiation therapy, offers the best chance for long-term survival.
    • Further research is needed to optimize treatment strategies for this rare malignancy.
