Bipolar disorder is a chronic mental health condition marked by significant mood fluctuations, including episodes of mania and depression. Elevated energy levels, heightened mood or irritability, impulsive behavior, reduced sleep needs, rapid speech, racing thoughts, inflated self-esteem, and distractibility characterize mania. Individuals with bipolar disorder often alternate between depressive and manic states, with periods of emotional stability lasting an average of six months to a year.

Background:

  • Bipolar disorder (BD) is marked by recurring mood episodes.
  • Sleep-wake and circadian rhythm disturbances are potential predictors of BD recurrence.
  • Few studies have objectively investigated these predictors.

Purpose of the Study:

  • To assess if actigraphy-derived sleep-wake and circadian rhythm features predict BD recurrence.
  • To determine if these objective measures improve prediction of early vs. late recurrence.

Main Methods:

  • Euthymic individuals with BD (n=69) underwent 3 weeks of actigraphy.
  • Participants were followed for a median of 3.5 years for recurrence.
  • Survival and AUC analyses explored associations between actigraphy, clinical variables, and recurrence.

Main Results:

  • 64% of participants experienced recurrence, with a median survival time of 18 months.
  • Circadian rhythm amplitude and variability/stability significantly predicted time to recurrence (p=0.009).
  • Combining actigraphy with clinical predictors improved AUC for early vs. late recurrence prediction from 0.64 to 0.82 (p=0.04).

Conclusions:

  • Actigraphy measures of circadian rhythm stability and amplitude are valid predictors of future BD recurrences.
  • These objective markers could inform targeted psychosocial interventions.
  • Objective circadian rhythm data enhances the prediction of BD recurrence timing.
