    Background:

    • Bronchial asthma diagnosis often relies on bronchoprovocation testing.
    • Histamine and methacholine are common agents used to assess airway hyperreactivity.

    Purpose of the Study:

    • To evaluate the diagnostic utility of histamine and methacholine bronchoprovocation tests in patients with suspected bronchial asthma.
    • To compare the airway hyperreactivity between allergic and nonallergic asthmatics.

    Main Methods:

    • Forty-eight patients with suspected asthma underwent histamine and methacholine challenge tests.
    • Pulmonary function tests, including FEV1 and FEF25-75, were measured.
    • Patients were categorized into groups based on test results and allergic status.

    Main Results:

    • 14% of patients had negative provocation tests.
    • 37% were classified as allergic asthmatics with positive challenges.
    • 34% were nonallergic asthmatics with positive challenges.
    • Allergic asthmatics demonstrated significantly higher sensitivity to both histamine and methacholine challenges (p < 0.02).
    • Methacholine showed greater sensitivity in detecting airway hyperreactivity in nonallergic asthmatics.

    Conclusions:

    • Bronchoprovocation tests with histamine and methacholine are valuable in diagnosing bronchial asthma.
    • Airway hyperreactivity patterns differ between allergic and nonallergic asthmatics.
    • Methacholine may be a more sensitive agent for nonallergic asthma detection.
