Summary

Intravenous immunoglobulin (IV Ig) therapy shows promise as a rescue treatment for severe, complicated dengue cases in adults. This study suggests IV Ig can be a valuable option when standard therapies fail in critically ill dengue patients.

Background:

  • Severe dengue leads to over 22,000 deaths annually, with complicated dengue having a high mortality rate (44-72%).
  • Hallmarks of complicated dengue include immune system dysregulation, capillary leak, and thrombocytopenia.
  • Intravenous immunoglobulin (IV Ig) is effective in pediatric severe dengue but its role in adults remains under-explored.

Purpose of the Study:

  • To evaluate the effectiveness of IV Ig as a rescue therapy in critically ill adult patients with complicated dengue.
  • To explore the immunoresuscitative role of IV Ig in severe dengue.

Main Methods:

  • A retrospective observational study was conducted in a tertiary care hospital in Southern India.
  • Data were collected from January 1, 2018, to December 31, 2019, for patients with complicated dengue treated with IV Ig.
  • IV Ig was administered at a dose of 0.4 g/kg for 5 days to critically ill patients who failed standard therapy.

Main Results:

  • Out of 999 dengue admissions, 40.24% presented with warning signs, and 12.11% had bleeding.
  • Severe complications included shock (10.21%), acute kidney injury (2.90%), and adult respiratory distress syndrome (2.40%), with an overall mortality of 0.40%.
  • IV Ig was administered to 13 critically ill patients, including those with refractory shock, encephalitis, and hemophagocytic lymphohistiocytosis. Two patients died.

Conclusions:

  • Intravenous immunoglobulin (IV Ig) therapy can serve as a valuable rescue option for critically ill patients with complicated dengue.
  • The findings support the use of IV Ig in severe dengue cases where conventional treatments are insufficient.
