Thyroid hemiagenesis, a rare thyroid development issue, can present with hypothyroidism, not just hyperthyroidism. This case highlights a 72-year-old woman with congenital thyroid hemiagenesis and lifelong hypothyroidism.

Background:

Thyroid hemiagenesis is a rare congenital anomaly characterized by the unilateral absence of a thyroid lobe.
It is more prevalent in females and often involves the left lobe and thyroid isthmus.
While frequently associated with hyperthyroidism, it can also present with hypothyroidism.

Observation:

This report details a 72-year-old female diagnosed with hypothyroidism 25 years prior.
Neck ultrasound revealed the absence of the left thyroid lobe and isthmus.
Laboratory results indicated controlled hypothyroidism on levothyroxine therapy.

Findings:

The patient was diagnosed with congenital thyroid hemiagenesis associated with hypothyroidism.
Thyroid hemiagenesis can be an incidental finding presenting later in life.
Patients with thyroid hemiagenesis may have reduced thyroid hormone reserves, increasing hypothyroidism risk.

Implications:

This case expands the clinical spectrum of thyroid hemiagenesis presentation.
It underscores the importance of considering hypothyroidism in patients with thyroid hemiagenesis.
Early diagnosis and management are crucial for patients with thyroid hemiagenesis and associated thyroid dysfunction.
