Summary

Immune evasion via PD-L1 is key in B-cell cancers. This study found rare extranodal diffuse large B-cell lymphoma (DLBCL, NOS) cases with tumor cell PD-L1 expression, some linked to 3'-UTR variations.

Background:

  • Programmed death-ligand 1 (PD-L1) mediates immune evasion in B-cell malignancies.
  • PD-L1 expression is uncommon in tumor cells of extranodal diffuse large B-cell lymphoma, not otherwise specified (DLBCL, NOS).

Purpose of the Study:

  • To investigate mechanisms of PD-L1 aberrant upregulation in extranodal DLBCL, NOS.
  • To identify cases of extranodal DLBCL, NOS exhibiting tumor cell PD-L1 expression.

Main Methods:

  • Case series analysis of extranodal DLBCL, NOS.
  • Investigation of PD-L1 expression in tumor cells.
  • Analysis of structural variations, particularly in the 3 -untranslated region (3 -UTR) of PD-L1.

Main Results:

  • Four cases of extranodal DLBCL, NOS with tumor cell PD-L1 expression were identified.
  • Two cases showed structural variations in the PD-L1 3 -UTR contributing to aberrant upregulation.
  • Two cases lacked identified structural variations in the PD-L1 3 -UTR.

Conclusions:

  • Aberrant PD-L1 upregulation in extranodal DLBCL, NOS can occur through mechanisms beyond copy number alterations, including structural variations in the 3 -UTR.
  • A subset of extranodal DLBCL, NOS cases may represent an "immune evasion-type" characterized by tumor cell PD-L1 expression.
