Purpose of the Study:

  • To assess the feasibility of integrating telepsychiatric triage into rural primary care.
  • To identify and mitigate unmet mental health needs upstream from emergency departments.

Main Methods:

  • A 6-month feasibility case study in a single midwestern rural county.
  • Patients at primary care visits were referred for on-site telepsychiatric triage if at risk for moderate-severe depression, self-harm, or other mental health concerns.
  • Patients identified with further concerns received up to six psychiatric and/or social work encounters for stabilization and treatment.

Main Results:

  • 68 patients were referred to telepsychiatric triage, representing 0.85% of the county's adult population.
  • Only two patients had a prior mental/behavioral health diagnosis.
  • 46 patients (67.6%) were diagnosed with at least one psychiatric disorder during the program.

Conclusions:

  • Telepsychiatric triage integrated into rural primary care is a feasible model.
  • This approach can aid in identifying and addressing unmet mental health needs in rural populations.
