Summary

Apparent diffusion coefficient (ADC) values effectively differentiate gynecological lesions. Gestational trophoblastic neoplasm (GTN) showed significantly higher ADC values than other uterine lesions, aiding diagnosis and management.

Background:

Purpose of the Study:

  • To evaluate the effectiveness of ADC values in differentiating various uterine lesions.
  • To compare ADC values across different gynecological pathologies, including endometrial cancer and gestational trophoblastic neoplasm (GTN).

Main Methods:

  • Retrospective analysis of 12 patients with uterine cervix carcinoma and 151 patients with uterine lesions.
  • Diffusion-weighted MRI with specific echo planar sequences was performed on all subjects.
  • ADC values were calculated for endometrial carcinoma, GTN, and other uterine lesions, with a control group of healthy volunteers.

Main Results:

  • Endometrial carcinoma exhibited significantly lower mean ADC values compared to all other groups (P < 0.001).
  • Gestational trophoblastic neoplasm (GTN) demonstrated significantly higher mean ADC values than all other uterine lesions (P < 0.001).
  • Statistically significant differences were observed in lesion-myometrium ADC ratios across the groups (P < 0.001).

Conclusions:

  • ADC values are effective in distinguishing between various uterine lesions.
  • GTN shows distinctively higher ADC values compared to other uterine pathologies.
  • These findings support the use of DWI in guiding the management of patients with GTN.
