Segmental endobronchial valve therapy for a vasculitis-induced emphysema

This study presents a case of emphysema linked to hypocomplementaemic urticarial vasculitis syndrome (HUVS) in a 53-year-old female. Endobronchial valve placement successfully treated the condition, improving lung function and clinical symptoms.

Background:
- A 53-year-old female with a history of hypocomplementaemic urticarial vasculitis syndrome (HUVS) and polyarteritis nodosa presented with exertional dyspnea.
- Lung function tests revealed severe obstructive lung disease with significant hyperinflation.
Observation:
- Multi-detector computed tomography (MDCT) and quantitative CT analysis confirmed lower lobe predominant emphysema.
- The emphysema was attributed to vasculitis due to the patient's age, disease history, and exclusion of alpha-1 antitrypsin deficiency.
Findings:
- MDCT identified severe emphysema and hyperinflation in the right lower lobe, specifically segment 8.
- Invasive Chartis® measurement showed no significant collateral ventilation in the isolated segment 8, leading to endobronchial valve placement.
Implications:
- Following endobronchial valve placement, MDCT showed complete collapse of segment 8.
- The intervention resulted in significant clinical benefit and a mild reduction in lung hyperinflation, suggesting a novel treatment approach for vasculitis-associated emphysema.
