Summary

Fixed drug eruption (FDE) is a skin reaction that reappears in the same spot when exposed to a drug again. This is the first reported case of FDE caused by the cancer immunotherapy drug atezolizumab.

Recognition of Epidermal Transglutaminase by IgA and Tissue Transglutaminase 2 Antibodies in a Rare Case of Rhesus Dermatitis

Background:

Fixed drug eruption (FDE) is a specific type of adverse cutaneous drug reaction.
FDE is characterized by recurrent lesions in the same location upon re-exposure to the causative agent.
Atezolizumab is an immune checkpoint inhibitor used in cancer therapy.

Observation:

A 48-year-old woman receiving atezolizumab for metastatic breast cancer developed FDE.
The patient experienced a fixed drug eruption.
This represents a novel association between atezolizumab and FDE.

Findings:

The case presents the first documented instance of FDE potentially induced by atezolizumab.
This finding expands the known spectrum of adverse drug reactions associated with immune checkpoint inhibitors.
Further investigation is warranted to confirm the causal link and understand the mechanism.

Implications:

Clinicians should be aware of FDE as a possible side effect of atezolizumab.
Early recognition and management of FDE can improve patient outcomes.
This case highlights the importance of monitoring for cutaneous reactions during immunotherapy.
