    Background:

    • Systemic lupus erythematosus (SLE) is a complex autoimmune disease with diverse clinical manifestations.
    • Monitoring disease activity is crucial for effective SLE management and treatment adjustment.

    Purpose of the Study:

    • To evaluate the utility of serum beta 2-microglobulin (beta 2-M) as a biomarker for assessing disease activity in patients with SLE.
    • To determine the correlation between beta 2-M levels and renal and extrarenal manifestations of SLE.

    Main Methods:

    • Serum beta 2-M levels were measured in 115 SLE patients on 192 occasions.
    • Creatinine clearance was assessed to evaluate renal function.
    • Clinical, serological, and renal activity were quantified using an arbitrary scoring system.

    Main Results:

    • Elevated beta 2-M was observed in 16.4% of patients with extrarenal SLE, 29.2% with renal SLE and normal creatinine clearance, and 65.7% with decreased creatinine clearance.
    • Beta 2-M levels were significantly higher in SLE patients compared to non-SLE glomerulonephritis patients with matched creatinine clearance.
    • Serum beta 2-M levels demonstrated a positive correlation with renal, extrarenal, and serological disease activity, and changes in beta 2-M reflected changes in clinical activity.

    Conclusions:

    • Serum beta 2-microglobulin is a valuable indicator of disease activity in systemic lupus erythematosus.
    • Beta 2-M can serve as a reliable biomarker for monitoring SLE progression and treatment response, especially in patients with renal involvement.
