    Cellular atypia in basal cell carcinoma (BCC) presents diagnostic challenges, mimicking other skin conditions. However, atypia in BCC does not appear to indicate more aggressive clinical behavior.

    • Basal cell carcinoma (BCC) is the most common human cancer.
    • Cellular atypia, a feature of malignancy, can be present in BCC.

    Observation:

    • Four cases of basal cell carcinoma with marked cellular atypia were identified in 2,000 specimens.
    • These lesions occurred on the sun-exposed head in middle-aged to older males.

    Findings:

    • The atypical BCCs showed significant variation, complicating differential diagnosis.
    • Distinguishing these from Bowen's disease and Darier's metatypical epithelioma was challenging.

    Implications:

    • Cellular atypia in BCC requires careful histopathological evaluation for accurate diagnosis.
    • Despite atypical features, atypia did not correlate with increased clinical aggressiveness in these cases.
