Localized necrotizing granulomatous lymphadenitis, a rare herpes simplex virus (HSV) presentation, was diagnosed in an immunocompromised patient with chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL). Diagnosis required lymph node biopsy, immunochemistry, and PCR testing.
