Background:

  • The link between complete atrioventricular (AV) block and Takotsubo syndrome (TTS) is recognized, but causality remains unclear.
  • Takotsubo syndrome, or broken heart syndrome, involves temporary weakening of the heart muscle.

Observation:

  • A 91-year-old female with a year-long untreated complete AV block developed Takotsubo syndrome.
  • Cardiac wall motion abnormalities were observed in the patient.

Findings:

  • Following permanent pacemaker implantation for the complete AV block, the patient's Takotsubo syndrome symptoms resolved.
  • The implanted pacemaker functioned normally during follow-up, and cardiac wall motion defects reversed.

Implications:

  • This case suggests a potential causal relationship where untreated complete AV block might precipitate Takotsubo syndrome.
  • Permanent pacemaker implantation may be a therapeutic option for Takotsubo syndrome in patients with concurrent complete AV block.
  • Further research is warranted to elucidate the pathophysiological mechanisms linking AV block and TTS.
