Summary

Periodic limb movements (PLM) and restless leg syndrome (RLS) are uncommon after stroke. This study highlights two cases of unilateral poststroke PLM, suggesting dopaminergic treatment may be effective for persistent symptoms.

Background:

  • Periodic limb movements (PLM) and restless leg syndrome (RLS) are typically idiopathic and bilateral.
  • Post-stroke PLM is rare, often unilateral, and frequently under-recognized due to transient symptoms.

Observation:

  • Two patients with unilateral post-stroke PLM are presented with differing clinical presentations, treatments, and outcomes.
  • Case 1: Chronic, long-lasting unilateral PLM with insomnia secondary to vascular lesions, responding to RLS treatment.
  • Case 2: Acute, self-limiting PLM with positive brain MRI findings.

Findings:

  • Unilateral post-stroke PLM can present distinctly from secondary RLS.
  • Long-lasting unilateral PLM may benefit from dopaminergic treatment, even in the absence of subjective RLS symptoms.
  • Chronic PLM in Case 1 may result from stroke-induced sensory pathway alterations.

Implications:

  • Highlights the potential for dopaminergic therapy in specific cases of chronic post-stroke PLM.
  • Suggests a need for increased recognition of unilateral post-stroke PLM.
  • Contributes to understanding the pathophysiology of secondary sleep-related movement disorders.
