Cerebral Venous Thrombosis in COVID-19

COVID-19 can cause cerebral venous thrombosis (CVT), sometimes as an early or presenting symptom, even in mild cases. This study found no significant difference in outcomes between COVID-19-associated CVT and non-COVID CVT patients.

- To compare the clinical characteristics, risk factors, and outcomes of CVT patients with and without COVID-19.

- A multicentric, retrospective study was conducted from April 4 to October 15, 2020.
- Adult patients with CVT, both SARS-CoV-2 positive (COVID-19 group) and negative (non-COVID group), were included.
- Clinical profiles, CVT risk factors, COVID coagulopathy markers, imaging, and outcome determinants were analyzed.
Main Results:
- The study included 18 COVID-19 patients and 43 non-COVID-19 CVT patients.
- CVT presented as the initial manifestation in 14 COVID-19 patients, with 13 having non-severe disease.
- Mortality and disability outcomes did not significantly differ between the COVID-19 and non-COVID-19 CVT groups.
Conclusions:
- A potential association between COVID-19 and CVT is suggested.
- CVT can be an early or presenting sign of COVID-19, even in mild infections.
- Poor outcomes in CVT patients were linked to lower Glasgow Coma Scale scores, abnormal chest CT, severe COVID-19, and need for ventilation.
