    Area of Science:

    • Ophthalmology
    • Gastroenterology
    • Rheumatology

    Background:

    • Crohn's disease is a chronic inflammatory bowel disease.
    • Systemic vasculitis is a known extraintestinal complication of Crohn's disease.
    • Retinal vasculitis associated with Crohn's disease is uncommon.

    Observation:

    • A 26-year-old woman with biopsy-proven Crohn's disease presented with severe bilateral retinal arteritis and phlebitis.
    • The condition led to significant vision impairment.

    Findings:

    • The patient experienced a marked loss of vision due to the obliterative nature of the retinal vasculitis.
    • The case highlights a rare but severe ocular manifestation of Crohn's disease.

    Implications:

    • Early recognition and intervention are crucial for managing retinal vasculitis in Crohn's disease patients.
    • Systemic corticosteroids and cyclophosphamide demonstrated efficacy in halting the progression of this rare complication.
    • This case underscores the importance of a multidisciplinary approach in managing complex Crohn's disease cases with ocular involvement.
