Purpose of the Study:

  • To evaluate a 3D virtual reality (VR) system for training interlaminar epidural access and SCS lead placement.
  • To assess the impact of VR simulation on trainee confidence and technical proficiency.

Main Methods:

  • Fourteen pain fellows and residents participated in the study.
  • Trainees underwent pre- and post-simulation surveys to gauge experience and confidence.
  • Performance was objectively measured using needle angle, the Pain Procedure Rating System (PaPRS), and procedure time during VR simulation sessions.

Main Results:

  • Significant improvements were observed in safe epidural space access (57.1% to 100%) and percutaneous lead delivery (7.1% to 71.4%).
  • Mean confidence levels increased by 71.4% for epidural access and 306% for SCS placement.
  • Procedural efficiency improved by 43.2%, and PaPRS scores increased by 73.3%.

Conclusions:

  • Virtual reality simulation is an effective tool for enhancing neuromodulation training.
  • VR-based didactics offer a viable solution to supplement variable clinical exposure in residency and fellowship programs.
  • This training modality can improve both the confidence and technical skills of future pain management specialists.
