Primary Healthcare Services

1.5K
Primary care promotes wellness and prevents disease. This care includes health promotion, education, protection (such as immunizations), early disease screening, and environmental considerations. Settings providing this type of healthcare include physician offices, public health clinics, school nursing, and community health nursing.
In 1978, international leaders convened in Alma-Ata, Kazakhstan, for what would be a pivotal event in global health. The Alma-Ata Declaration was the first to call...

The physical assessment examines the patient for objective data that defines the patient's condition, and aids in formulating the nursing care plan. The purpose of physical assessment is a health status appraisal, which includes identifying health problems, and establishing a database for nursing intervention.
The principles to begin the physical assessment include conducting a comprehensive or problem-related history in a quiet, well-lit room, emphasizing privacy and comfort for the...

Methods of Documentation V: CBE

997
Charting by Exception, or CBE, is a method of documentation used in healthcare, particularly in nursing, that focuses on documenting only significant or abnormal findings rather than recording every detail. This approach aims to streamline the documentation process, improve efficiency, and ensure that healthcare providers can quickly identify deviations from normalcy in patient assessments.
In CBE, healthcare professionals establish predefined standards of practice that define what constitutes...

Methods of Documentation II: POMR

1.1K
The Problem-Oriented Medical Record (POMR) revolutionized medical record-keeping by introducing a systematic approach focusing on the patient's problems rather than merely listing symptoms. Dr. Lawrence Weed's introduction of this method in the 1960s marked a significant advancement in medical documentation. The POMR framework consists of four key components: the database, problem list, plan of care, and progress notes.

Methods Of Healthcare Delivery System

3.5K
At the different levels of the healthcare system, we see varying methods of healthcare used. These methods include managed care systems, case management, and primary healthcare.
Managed Care System:
The managed care system is designed to control the cost while maintaining the quality of care. The patient's care from admission to discharge is planned by the primary care provider or the case manager, also known as the gatekeeper. In a managed care system, the number of care providers is...

Data Collection by Observations

12.9K
Data collection refers to a systematic way of obtaining, observing, measuring, and analyzing accurate information. Observational studies are one of the most widely used methods of data collection. It involves collecting data by observing the behavior and physical characteristics of a sample without making any modifications to the sample.
An astronomer viewing the motion and brightness of stars in the sky and recording the data is an example of observational data collection. A botanist recording...

Area of Science:

  • Primary Care Research
  • General Practice
  • Health Services Research

Background:

  • Primary care is expanding in China, but a lack of evidence exists on general practitioner (GP) activities and service delivery in Beijing.
  • Understanding the daily work of GPs is crucial for effective healthcare system development.

Purpose of the Study:

  • To explore the diverse work content and activities of general practitioners (GPs) within Beijing's primary care system.
  • To provide empirical data on GP roles, patient encounters, and associated tasks.

Main Methods:

  • A multi-method approach combining non-participant observation and critical incident technique interviews.
  • Observation of 1,100 patient encounters across five community health service institutions involving 11 GPs.
  • Interviews focused on out-of-clinic tasks and challenges faced by GPs.

Main Results:

  • GPs managed an average of 1.72 reasons for encounter and 2.51 health problems per patient visit.
  • Consultations primarily focused on disease diagnosis and treatment, with limited physical examinations (15.5%) and investigations (17.1%).
  • Chronic disease management procedures were infrequently provided (0.4-26.6%), and GPs engaged in various administrative, educational, and research tasks alongside clinical duties.

Conclusions:

  • GP work in Beijing is characterized by complexity and a heavy workload, encompassing clinical, administrative, and educational responsibilities.
  • There is a clear need for enhanced support systems and strategies to optimize GP performance and alleviate workload pressures in China's primary care sector.
