Background:

Acute epiglottitis, though rare post-vaccination for Haemophilus influenzae, is a critical consideration in pediatric upper airway obstruction.
Differential diagnosis is essential for timely and appropriate management of severe respiratory distress in children.

Observation:

A 3-year-old child presented with fever, respiratory distress, and stridor, indicative of acute epiglottitis.
The patient experienced ventilatory failure, necessitating endotracheal intubation and antibiotic therapy.

Findings:

Bacterial culture results confirmed Streptococcus pyogenes as the causative agent.
This case underscores that bacterial pathogens other than Haemophilus influenzae can cause acute epiglottitis.
