Background:

Colorectal adenocarcinoma can metastasize to unusual sites.
Thyroid metastasis from colorectal cancer is exceptionally rare, accounting for 0.1% of surgical series.

Observation:

A 62-year-old woman with a history of resected colon cancer presented with a growing, symptomatic right thyroid nodule.
Imaging showed extracapsular spread, and cytology confirmed metastatic adenocarcinoma.

Findings:

Histopathological review confirmed the thyroid nodule was metastatic adenocarcinoma originating from a colorectal primary.
The patient underwent a complex thyroidectomy and neck dissection.

Implications:

This case underscores the importance of considering rare metastatic sites in patients with a history of colorectal cancer.
Early recognition and diagnosis of thyroid metastasis are crucial for appropriate treatment planning.
Surgical management of thyroid metastases can present unique technical challenges.
