Ocrelizumab significantly reduces relapses and disability progression in multiple sclerosis (MS) patients. While effective, it may increase adverse events in primary progressive MS (PPMS) but is generally well-tolerated.

Background:

  • Ocrelizumab is a humanized anti-CD20 monoclonal antibody for multiple sclerosis (MS).
  • It is the only disease-modifying therapy approved for primary progressive multiple sclerosis (PPMS).

Purpose of the Study:

  • To assess the benefits, harms, and tolerability of ocrelizumab in relapsing-remitting MS (RRMS) and PPMS.
  • Evaluate ocrelizumab's efficacy and safety compared to placebo or interferon beta-1a.

Main Methods:

  • Systematic review of randomized controlled trials (RCTs) identified through comprehensive database searches.
  • Included adults with RRMS or PPMS meeting McDonald criteria, treated with ocrelizumab 600 mg every 24 weeks.

Main Results:

  • For RRMS, ocrelizumab showed reduced relapse rates and disability progression compared to interferon beta-1a, with similar adverse event rates.
  • For PPMS, ocrelizumab demonstrated reduced disability progression but a higher rate of adverse events compared to placebo.
  • Ocrelizumab treatment led to fewer gadolinium-enhancing T1 lesions and new T2 lesions in RRMS patients.

Conclusions:

  • Ocrelizumab effectively reduces relapses and disability progression in RRMS and PPMS.
  • While generally well-tolerated, ocrelizumab is associated with increased adverse events in PPMS patients.
  • Common adverse events include infusion reactions, nasopharyngitis, and respiratory/urinary tract infections.
