    Summary

    Textured breast implants showed no significant difference in malposition compared to smooth implants. However, using a polydioxanone (PDO) support matrix with smooth implants significantly reduced scar malposition in breast augmentation.

    Background:

    • Textured breast implants are utilized in aesthetic surgery to reduce implant malposition.
    • A recent analysis indicated similar malposition rates between smooth and micro-textured breast implants.
    • The prophylactic use of a polydioxanone (PDO) internal support matrix was explored to enhance pocket control and prevent scar malposition.

    Purpose of the Study:

    • To evaluate the efficacy of a polydioxanone (PDO) internal support matrix in preventing scar malposition during breast augmentation.
    • To compare implant-related complication and scar malposition rates among smooth implants, micro-textured implants, and smooth implants with PDO support.

    Main Methods:

    • A cohort of 200 patients undergoing silicone gel primary breast augmentation by a single surgeon was analyzed.
    • Patients were divided into three groups: smooth implants alone, micro-textured implants, and smooth implants with a PDO internal support matrix.
    • Implant-related complications and scar malposition were recorded and compared across the groups.

    Main Results:

    • No statistically significant difference in implant-related complication rates was observed between smooth and textured implants.
    • Scar malposition rates did not significantly differ between smooth and textured implant groups.
    • The group receiving smooth implants with a prophylactic PDO internal support matrix demonstrated the lowest scar malposition rate (4.48%), which was significantly lower than the smooth implant alone group (P=0.03).

    Conclusions:

    • Micro-textured implants showed a trend towards decreased scar malposition, though not statistically significant.
    • Prophylactic use of a PDO internal support matrix in silicone gel breast augmentation is safe.
    • The PDO internal support matrix significantly reduced scar malposition rates in breast augmentation procedures.
