Early Trifecta bioprosthesis failure is more common than with the Carpentier-Edwards Perimount Magna Ease valve. Prosthetic valve aortic regurgitation and structural valve deterioration drive Trifecta valve failure, necessitating reoperation.
Area of Science:
- Cardiovascular Surgery
- Biomaterials Science
- Medical Device Engineering
Background:
- Increasing reports of early Trifecta bioprosthesis failure.
- Need to compare hemodynamic performance of Trifecta and Carpentier-Edwards Perimount Magna Ease valves.
Purpose of the Study:
- Determine the failure mechanism of externally mounted pericardial Trifecta bioprosthesis.
- Compare early hemodynamic performance and failure rates between Trifecta and Carpentier-Edwards Perimount Magna Ease valves.
Main Methods:
- Retrospective study of 270 patients undergoing aortic valve replacement (2014-2021).
- Compared Trifecta (N=137) and Carpentier-Edwards Perimount Magna Ease (N=133) valve patients.
- Mean follow-up of 48.0 months.
Main Results:
- Higher incidence of major aortic regurgitation in Trifecta valves (6.3% vs. 0%, P<0.009).
- Structural valve deterioration occurred in 33% of Trifecta failures, leading to redo aortic valve replacement.
- Significantly lower freedom from redo aortic valve replacement due to structural valve deterioration at 5 years for Trifecta (89.4% vs. 100%, P=0.003).
- Prosthetic valve aortic regurgitation was a sole contributor to redo aortic valve replacement (HR: 2.38).
Conclusions:
- Trifecta valves exhibit significantly more early failures compared to Carpentier-Edwards Perimount Magna Ease valves.
- Prosthetic valve aortic regurgitation and calcific structural valve deterioration synergistically contribute to Trifecta valve failure.
- Trifecta valves demonstrated a reasonable mean transprosthetic pressure gradient over time.

Heart Valves
The AV valves prevent the backflow of blood from the ventricles to the atria during ventricular contraction. These valves function with the assistance of the chordae tendineae and papillary muscles. When the ventricles are relaxed, the chordae tendineae are slack, allowing blood to flow from the atria into the...
