    Background:

    Observation:

    • Two female patients, aged 18 and 17, presented with essential myoclonus.
    • Both patients showed normal functioning after treatment.

    Findings:

    • Physiologic analysis indicated ballistic movement overflow and myoclonus sensitive to sound stimuli.
    • Pharmacologic studies revealed a direct antagonism between physostigmine and anticholinergic agents, suggesting cholinergic hyperactivity.

    Implications:

    • Cholinergic hyperactivity is implicated in the pathophysiology of essential myoclonus.
