Parathyroidectomy after renal transplantation.

    Summary

Parathyroidectomy after kidney transplant is rare, even with a liberal surgical approach. Mild hyperparathyroidism post-transplant may not warrant surgery or extensive diagnostics.

    Area of Science:

    • Nephrology
    • Endocrinology
    • Transplantation Surgery

    Background:

    • Patients with advanced renal failure often develop hyperparathyroidism.
    • Persistent hyperparathyroidism can complicate kidney transplantation outcomes.

    Purpose of the Study:

    • To evaluate the incidence and necessity of parathyroidectomy after renal transplantation.
    • To question the rationale for surgical intervention in mild post-transplant hyperparathyroidism.

    Main Methods:

    • Retrospective analysis of 569 renal transplant recipients over 12 years.
    • Identification of patients undergoing subtotal parathyroidectomy post-transplantation.

    Main Results:

    • Eleven patients (1.9%) required parathyroidectomy after transplantation.
    • Hyperparathyroidism signs appeared within months to years post-transplant.
    • Four patients had bone/soft tissue calcifications; seven had only elevated calcium.

    Conclusions:

    • Subtotal parathyroidectomy is infrequent even with a liberal post-transplant surgical policy.
    • The need for surgical treatment of mild hyperparathyroidism and advanced diagnostics post-transplant is questionable.
