    Summary

    Strangulation in infants can cause severe brain injury, including cerebral infarction and subdural hematomas, even without visible head trauma. These findings on computed tomography (CT) scans may indicate child abuse.

    Background:

    • Child abuse frequently impacts the central nervous system.
    • Strangulation is a recognized but less commonly documented mechanism of abusive head trauma.

    Purpose of the Study:

    • To describe the neuroimaging findings and clinical outcomes in infants suffering strangulation-induced brain injury.
    • To highlight the diagnostic utility of computed tomography (CT) in identifying non-accidental trauma.

    Main Methods:

    • Retrospective case series of three infants presenting with strangulation injuries.
    • Review of clinical presentations, computed tomography (CT) findings, and autopsy results (in one case).

    Main Results:

    • All three infants presented with large cerebral infarctions and small subdural hematomas on CT.
    • No history or external signs of significant head trauma were present.
    • Autopsy confirmed hemispheric infarct, subdural hematoma, and carotid artery subintimal hemorrhage in one infant.

    Conclusions:

    • CT findings of cerebral infarction and subdural hematoma in infants without trauma history should raise suspicion for child abuse.
    • Strangulation can be the primary mode of injury and a presenting manifestation of abuse.
