Background:

Appendicitis typically presents with right lower quadrant pain.
Intestinal malrotation, particularly non-rotation, can alter appendiceal presentation.
Left-sided appendicitis is uncommon and diagnostically challenging.

Observation:

A 40-year-old female presented with left lower abdominal pain and localized peritonitis.
Undiagnosed intestinal non-rotation was identified as the underlying condition.
Imaging revealed left-sided appendicitis in the context of intestinal non-rotation.

Findings:

Surgical intervention included diagnostic laparoscopy, explorative laparotomy, and appendectomy.
The case highlights the diagnostic difficulties posed by atypical appendicitis presentations.
Multidetector computed tomography (MDCT) and ultrasonography were key in diagnosis.

Implications:

Clinicians must consider left-sided appendicitis in patients with left lower abdominal pain, especially with suspected intestinal malrotation.
Prompt diagnosis through advanced imaging is essential for timely intervention.
This case underscores the importance of recognizing rare presentations of common diseases.
