Ankle Joint01:10

Ankle Joint

The ankle is formed by the talocrural joint (crural = leg). It consists of the articulations between the talus bone of the foot and the distal ends of the tibia and fibula of the leg. The superior aspect of the talus bone is square-shaped and has three areas of articulation. The top of the talus articulates with the inferior tibia. This is the portion of the ankle joint that carries the body weight between the leg and foot. The sides of the talus are firmly held in position by the articulations...

Area of Science:

  • Orthopedic Surgery
  • Traumatology
  • Sports Medicine

Background:

  • Complete isolated calcaneal dislocation is an extremely rare injury involving the talocalcaneal and calcaneocuboid joints.
  • This specific type of dislocation is characterized by an intact talonavicular joint and minimal associated fractures.

Observation:

  • A 49-year-old male presented with a closed isolated anterolateral calcaneal dislocation following a motor vehicle collision.
  • The injury involved fractures of the sustentaculum tali, cuboid, lateral talar process, and a superior peroneal retinaculum avulsion fracture.

Findings:

  • Urgent closed reduction was performed, followed by surgical management including peroneal tendon and retinaculum repair, chondroplasty, microfracture, and fixation of the calcaneocuboid joint.
  • Subsequent staged fixation of the talocalcaneal joint and sustentaculum tali addressed associated fractures.
  • The patient achieved full weight-bearing by 10 weeks post-injury, returning to regular activities within 6 months.

Implications:

  • Prompt diagnosis and appropriate surgical intervention are crucial for managing isolated calcaneal dislocations.
  • Even complex cases with multiple associated fractures can achieve favorable functional outcomes and patient satisfaction.
