Background:
- Ductal epithelial changes, specifically lympho-epithelial lesions (LEL), in the context of prostatic chronic inflammation (CI) have been understudied.
- Understanding these lesions is crucial for a comprehensive view of the inflammatory microenvironment within the prostate.
- LEL may play a role in the pathogenesis or progression of various prostatic conditions.
Purpose of the Study:
- To investigate the presence and characteristics of lympho-epithelial lesions (LEL) immediately adjacent to prostatic chronic inflammation (CI).
- To determine the association of LEL with different types and grades of prostatic inflammation.
- To assess the expression of PD-L1 in LEL within the prostate.
Main Methods:
- Analysis of 144 prostatic surgical and autopsy specimens.
- Inclusion of various types of prostatic CI: NIH-category IV prostatitis (histologic prostatitis-HP), nonspecific granulomatous prostatitis (NSGP), and reactive lymphoid infiltrates near benign prostatic hyperplasia (BPH) and prostate adenocarcinoma (PCa).

- PD-L1 positive LEL are consistently found in high-grade chronic prostatic inflammation but lack organ specificity.
- LEL are integral components of prostate-associated lymphoid tissue (PALT) and the inflammatory prostatic microenvironment.
