    Summary

    Immunofluorescence (IF) revealed minimal immunoglobulin deposits in leprosy skin lesions. Complement was found in various skin structures, and autofluorescing macrophages were present in lepromatous leprosy.

    Background:

    • Leprosy, a chronic infectious disease, affects the skin and peripheral nerves.
    • Understanding the immune response in leprosy skin lesions is crucial for diagnosis and treatment.
    • Immunofluorescence (IF) is a valuable tool for visualizing immune deposits in tissue.

    Purpose of the Study:

    • To investigate the distribution and types of immune deposits in skin lesions of leprosy patients.
    • To correlate immunofluorescence findings with the leprosy spectrum and treatment status.

    Main Methods:

    • Examined 40 skin biopsies from untreated and treated leprosy patients across the disease spectrum.
    • Utilized immunofluorescence (IF) methods with fluorescein isothiocyanate (FTC)-labelled antihuman immunoglobulin antisera.
    • Assessed deposits along the basement membrane, in vessel walls, around skin appendages, and in macrophages.

    Main Results:

    • Minimal or no immunoglobulin staining was observed in lesional skin across all patients.
    • Sparse IgM deposits were noted in some borderline lepromatous and lepromatous patients.
    • Complement deposits were found in the epidermal basement membrane and vessel walls (tuberculoid and lepromatous).
    • Complement and some IgG deposits were observed around sweat glands and skin appendages.
    • Autofluorescing macrophages were present in lepromatous leprosy, with complement deposits in about half.

    Conclusions:

    • Immunoglobulin deposits are generally absent or minimal in leprosy skin lesions.
    • Complement deposition is a more consistent finding, present in various skin structures and associated with different leprosy forms.
    • Autofluorescing macrophages and associated complement deposits may indicate specific immune responses in lepromatous leprosy.
