  • Retrospective cross-sectional study using electronic health record data from a National Cancer Institute Cancer Center.
  • Included patients with lung cancer receiving systemic therapy in 2019.
  • Patient-level adherence defined as PRO completion within 30 days of treatment visits; multivariable regression used to identify predictors (P < .05).

  • 1,105 patients with lung cancer had 18,604 encounters in 2019.
  • Mean patient-level PRO adherence was 49.4%, ranging from 27.2% to 70.0% across sites.
  • Advanced age (≥ 65 years) and Black race were negatively associated with PRO adherence (P < .01).

  • Real-world adherence to PRO monitoring in lung cancer patients is lower than in clinical trials.
  • Potential age- and race-based disparities in PRO adherence were identified.
  • Standardized reporting of adherence metrics can help address this implementation gap.
