Background:

Invasive fungal infections, particularly mucormycosis, are underrecognized complications following traumatic injuries.
Trauma can lead to the inoculation of environmental fungal spores into normally sterile body sites, rapidly causing severe disease.

Purpose of the Study:

To present a case series of four trauma patients with invasive fungal infections.
To develop a work-up and treatment algorithm for post-trauma invasive fungal infections.

Main Methods:

Case series of four trauma patients diagnosed with mucormycosis across two hospitals.
Review of clinical presentations, management strategies, and patient outcomes.

Main Results:

Three of the four patients survived after medical and surgical interventions; one patient died.
Survivors had lower extremity infections, while the deceased patient had extensive thoracic involvement.
Mucormycosis in trauma patients may present atypically, requiring high clinical suspicion.

Conclusions:

Mucormycosis is a rare but significant post-trauma complication with high morbidity and mortality.
Prompt diagnosis and intervention, including surgical debridement and antifungal therapy, are critical.
Consideration of novel treatments like antifungal irrigation may be beneficial.
