Endoscopic Procedures IV: Sigmoidoscopy and Laproscopy

187
Sigmoidoscopy and laparoscopy are distinct medical procedures that enable physicians to internally inspect different parts of the GI tract. Although they serve different purposes, each is essential for diagnosing and, in some cases, treating various medical conditions.
Sigmoidoscopy
Sigmoidoscopy is a diagnostic procedure that uses a flexible sigmoidoscope equipped with a light source and camera to examine the rectum and sigmoid colon. The procedure involves inserting the tube through the anus...

Summary

Laparoscopic sleeve gastrectomy (LSG) requires approximately 25 patients to master. Surgical skill improves across three learning curve phases, reducing operative time and gastric stenosis without impacting complications or weight loss.

Area of Science:

  • Bariatric Surgery
  • Obesity Treatment
  • Surgical Education

Background:

  • Bariatric surgery is the most effective treatment for morbid obesity.
  • Laparoscopic sleeve gastrectomy (LSG) is a popular bariatric procedure.
  • Objective analysis of the LSG learning curve is lacking.

Purpose of the Study:

  • To analyze the learning curve for laparoscopic sleeve gastrectomy (LSG).
  • To identify distinct phases of surgical skill acquisition in LSG.

Main Methods:

  • Cumulative sum (CUSUM) analysis was used to evaluate the LSG learning curve.
  • Data from LSG procedures performed between April 2013 and February 2016 were analyzed.
  • Surgical outcomes were compared across different learning curve phases.

Main Results:

  • The LSG learning curve was divided into three phases: early learning (26 patients), skill acquisition (30 patients), and mastery (56 patients).
  • Operative time and gastric stenosis rates significantly decreased with learning curve progression.
  • No significant differences were observed in 30-day complication rates, postoperative stay, or weight loss.

Conclusions:

  • The learning curve for LSG can be delineated into three distinct phases.
  • Approximately 25 patients are needed to achieve demonstrable improvement in LSG surgical skill.
  • Mastery of LSG technique enhances efficiency and reduces complications.
