[Asymptomatic hyperparathyroidism. An indication for surgery?].

    Summary

Early surgery for asymptomatic primary hyperparathyroidism (pHPT) is recommended. Surgical intervention normalizes key health markers and improves patient well-being, preventing long-term complications.

    Area of Science:

    • Endocrinology
    • Surgical Oncology
    • Metabolic Disorders

    Context:

    • Primary hyperparathyroidism (pHPT) is a common endocrine disorder.
    • A subset of pHPT patients present without overt symptoms.
    • Long-term consequences of untreated pHPT can be significant.

    Purpose:

    • To evaluate the outcomes of surgical intervention in asymptomatic primary hyperparathyroidism.
    • To assess the impact of surgery on biochemical parameters and patient health.
    • To determine the long-term benefits of early surgical treatment for pHPT.

    Summary:

    • A study reviewed 412 patients operated on for primary hyperparathyroidism (pHPT) between 1965 and 1985.
    • 35 patients (8.7%) were asymptomatic at diagnosis.
    • Follow-up revealed normalization of alkaline phosphatase, uric acid, creatinine, and hypertension, alongside improved general condition, with all patients achieving normocalcemia.

    Impact:

    • Early surgical treatment for asymptomatic pHPT is beneficial.
    • Surgery effectively resolves biochemical abnormalities associated with pHPT.
    • Proactive surgical management can prevent the development of serious long-term complications in pHPT patients.
