Summary

Dasatinib treatment can cause a novel Crohn's-like colitis. This drug-induced intestinal injury may present with microgranulomas, potentially mimicking Crohn's disease on biopsy.

  • Dasatinib, a multityrosine kinase inhibitor, treats Philadelphia chromosome-positive leukaemia.
  • Common Dasatinib-induced intestinal injuries include haemorrhagic colitis, apoptotic colopathy, and CD8+ T-cell-mediated colitis.
  • A novel histopathologic pattern of Dasatinib-induced colitis has not been previously described.

Purpose of the Study:

  • To describe a novel Crohn's-like histopathologic pattern of Dasatinib-induced colitis.
  • To differentiate this pattern from other forms of drug-induced colitis and Crohn's disease.

Main Methods:

  • Case series of four patients treated with Dasatinib who developed diarrhoea.
  • Colonoscopy with biopsy for histological analysis.
  • Histological examination of ileal and rectal biopsies.

Main Results:

  • All four patients exhibited patchy, chronic active inflammation with cryptitis and microgranulomas (in two patients).
  • Ileal and rectal biopsies showed mild, focal inflammation.
  • Increased lamina propria eosinophils (two patients) and apoptoses (three patients) were observed.
  • Complete remission occurred after discontinuing Dasatinib.

Conclusions:

  • Dasatinib can induce a unique Crohn's-like colitis.
  • Histologic features, including microgranulomas, can overlap with Crohn's disease.
  • Clinical information regarding Dasatinib treatment is crucial to avoid misdiagnosis.
