Summary

Noninvasive ventilation (NIV) and invasive mechanical ventilation (IMV) showed similar effects on inflammation. However, NIV significantly reduced local lung inflammation in rats with lung injury, unlike IMV.

Background:

  • The inflammatory impact of noninvasive ventilation (NIV) versus invasive mechanical ventilation (IMV) is not well understood.
  • Quantifying local and systemic inflammation is crucial for comparing respiratory support strategies.

Purpose of the Study:

  • To compare the effects of NIV and IMV on local and systemic inflammation in rats with lipopolysaccharide (LPS)-induced lung injury.
  • To evaluate inflammatory responses in healthy rats under NIV and IMV.

Main Methods:

  • Rats with LPS-induced lung injury and healthy rats were subjected to 4 hours of NIV, IMV, or control (spontaneously breathing).
  • Cytokine levels in serum and lung tissue were measured using multiplex enzyme-linked immunosorbent assay.

Main Results:

  • No significant differences in serum or lung cytokine levels were observed between NIV and IMV groups in either LPS-injured or healthy rats.
  • In LPS-injured rats, NIV treatment significantly reduced pulmonary levels of IL-1α, IL-6, and TNF-α compared to controls.
  • IMV treatment did not show a significant reduction in these pulmonary cytokines compared to controls in LPS-injured rats.

Conclusions:

  • Noninvasive ventilation, unlike invasive mechanical ventilation, demonstrated an ability to attenuate local pulmonary inflammation in an LPS-induced lung injury model.
  • NIV may offer a localized anti-inflammatory benefit in certain lung injury scenarios.
