This review of kinematic rotating-hinge total knee arthroplasties found a high complication rate, including sepsis and instability. The prosthesis is recommended only for severe cases with absent collateral ligaments.

    Area of Science:

    • Orthopedic Surgery
    • Biomedical Engineering

    Background:

    • Review of the first fifty kinematic rotating-hinge total knee arthroplasties performed at the Mayo Clinic.
    • Indication for prosthesis: ligamentous instability, bone loss, or both.

    Observation:

    • Follow-up of 38 knees (36 patients) at a mean of 50 months.
    • 15 primary and 23 revision arthroplasties evaluated.
    • Clinical results: 14 excellent, 12 good, 5 fair, 5 poor.

    Findings:

    • Radiographic findings: 25% femoral and 50% tibial component lucency.
    • Progression of lucency in 13 knees; probable loosening in 5.
    • High complication rates: 16% sepsis, 22% patellar instability, 6% implant breakage.

    Implications:

    • The kinematic rotating-hinge prosthesis is associated with a significant complication rate.
    • Consideration for use limited to knees with functional absence of collateral ligaments unresponsive to reconstruction.
