    Background:

    • Pirprofen, a non-steroidal anti-inflammatory drug (NSAID), is used for pain and inflammation.
    • Adverse drug reactions require continuous monitoring and reporting.

    Observation:

    • Four cases of severe hepatic injury linked to pirprofen use were reported.
    • Patients developed liver damage between 3.5 and 6.5 months of daily pirprofen treatment (400-1200 mg).
    • Histological examination revealed acute hepatocellular damage with bridging necrosis.

    Findings:

    • Two patients died from pirprofen-induced liver injury.
    • The remaining two patients experienced incomplete recovery.
    • The reaction appears rare, unpredictable, and not associated with immunoallergic signs, suggesting metabolic idiosyncrasy.

    Implications:

    • Healthcare providers should be aware of the potential for severe hepatotoxicity with pirprofen.
    • Further investigation into the metabolic pathways of pirprofen is warranted.
    • This highlights the importance of pharmacovigilance for NSAIDs.
